The Omega Christopher (TOC) is a community-based organization dedicated to improving health outcomes for older adults by addressing the everyday factors that impact well-being. Serving individuals aged 60 and older in the South Suburbs of Chicago, TOC focuses on reducing chronic disease risk through food as medicine, behavioral reinforcement, and meaningful social engagement. The organization targets key barriers such as food insecurity, social isolation, and limited access to consistent support systems.
TOC works alongside healthcare providers to strengthen—not replace—clinical care by helping participants stay engaged in nutrition, wellness, and preventive services. Through structured programming and community partnerships, TOC creates sustainable lifestyle support systems that empower individuals to better manage conditions like diabetes, hypertension, and cardiovascular disease. 8-Week Wellness Cohorts

Health Education & Condition Management
